Media Release Agreement

By clicking here, I grant irrevocable permission to the Utah Education Association and its agents, employees, licensees, or assigns (collectively “UEA”) to use all information provided herein for any lawful purpose in any form or media. I further grant UEA the nonexclusive royalty-free, transferable (by assignment, license or otherwise), irrevocable right and license to create copyrightable derivative works from this information to use, display, reproduce, publish, transmit, and distribute the information, in whole or in part, for any lawful purpose in any form or media. I further waive any and all rights that I might have to royalties, compensation, remuneration, and/or profit relating to the use of the information identified above and release and discharge UEA from any and all claims and demands arising out of or in connection with the use thereof.  I also waive any right to inspect or approve the finished media in which they are used, now or in the future, whether the use is known to me or unknown.

I am 18 years of age or older and I am competent to contract in my own name. I have read this release before agreeing, and I fully understand the contents, meaning and impact of this release. I understand that I am free to address any specific questions regarding this release by submitting those questions in writing prior to agreeing, and I agree that my failure to do so will be interpreted as a free and knowledgeable acceptance of the terms of this release.
